Disk Jockey (DJ) Contract

 

This Agreement, made this _________________________________ (today’s date) by and between James A. Carratt Jr., hereafter referred to as Seller, and ______________________________ (your Name), hereafter referred to as Purchaser, of ____________________________________________________________________ (your address) and can be reached at (        )_______ - ______________ (your phone number).  The Seller agrees to provide DJ services for the Purchaser on:

 The Seller agrees to provide to the Purchaser, DJ services in consideration of the sum total of ____________________________________($____________). 

 

The Following Conditions Apply:

1.             DJ Services shall be paid in full prior to the start of the engagement and/or festivities.  All payments on the day of the engagement shall be made in cash, cashiers check, or money order and shall be made payable to James A. Carratt Jr.

2.             No Refunds of deposits will be made to the Purchaser if the engagement is broken off and/or cancelled by the Purchaser and/or a representative of the Purchaser.

3.             If the Seller is unable to perform services and/or cancels, the Purchaser is entitled to full reimbursement of the deposit mentioned below. 

4.             It is hereby agreed between both parties that J&J Mobile DJ, Bill Flory, their employees, administrators heirs and

                 successors cannot be held responsible any claims, actions, suits and/or litigation arising from the performance at the

                above mentioned location.

5.             If the Seller is unable to perform services and/or cancels within 7 days of the above-mentioned engagement, the Purchaser is entitled to full reimbursement of his/her deposit plus a $50.00 penalty applied against the Seller and made payable to the Purchaser.  It is further agreed that no additional penalties can be applied to the Seller above and beyond $50.00. 

6.             The Deposit for this engagement is $200.00.  The remaining balance due must be paid in full prior to the engagement.

7.             At no time shall the Purchaser, and or a representative of the Purchaser require the Seller to perform longer than the above mentioned times without further consideration.  Should services be required beyond the hours indicated above, the Seller is entitled to $100.00/hour payable to the Seller prior to the commencement of additional services.  If the Seller is unable to perform beyond the contracted hours, no refunds shall be made to the Purchaser.

8.             The seller cannot be held responsible for improper directions, unknown name of establishment and/or inadequate power supply from the location of the performance.

9.             The Seller agrees to provide:

a.             Speakers and Lighting from Package ____ (Please indicate your selection)

b.             CD’s with a variety of music.  It is hereby agreed between both parties that Disk Jockey Services provided by the Seller are for a variety of music (Hip-hop, Country, Dance, Rock, Disco, Oldies and etc.) and that not one type of music will be made exclusive throughout the night.

c.             CD Recordings of karaoke participants.  It is further agreed between the Seller and Purchaser that the CD Recordings are not guaranteed due to disk errors and etc.  CD Recording services are provided as an additional service.  James A. Carratt Jr. and/or any other representative of J&J Mobile DJ cannot be held liable, nor shall refunds be provided from the Seller to the Purchaser, for errors involved with CD recordings.

d.             The Seller will be in professional attire for the engagement. (Dress Slacks, Dress Shirt and Tie)

10.           The Seller has received from the Purchaser $_________ this _________________________ (today’s date).  The Purchaser is aware of all conditions regarding deposits as mentioned above.  The balance due prior to the commencement of Disk Jockey services is $___________.  The balance due shall not be construed as part of the deposit, nor shall it be withheld if paid in advance.  Should the Purchaser cancel the engagement the Seller is entitled to retain only the $200.00 initial deposit.

11.           No refunds shall be made once commencement of services has begun unless cancelled by the Seller

 

This engagement shall be binding upon the parties, their successors, assigns and personal representatives.  Time is of the essence on all undertakings.  This agreement shall be in force under the laws of the State of Illinois
